Knowledge and Attitude of Primary Care Physicians towards Palliative Care in Kuwait

"تلعب مؤسسة الكويت للتقدم العلمي دورا كبيرا في زيادة الوعي بأهمية الرعاية التلطيفية في الكويت. لذا أجرينا البحث الحالي الذي يهدف إلى زيادة الوعي والمعرفة لدى أطباء الرعاية الأولية في الكويت عن مرضى الرعاية التلطيفية. في منطقة الخليج، حتى وقت قريب، كانت الرعاية التلطيفية يمثلها عدد قليل نسبيا من الوحدات المتخصصة المستندة إلى المستشفيات. وفي الكويت، لا تزال خدمات الرعاية التلطيفية في مرحلة مبكرة من التطور. ويحاول العديد من مقدمي الرعاية الأولية تجنب التعامل مع هؤلاء المرضى على الرغم من أنهم يستخدمون كأطباء الخط الأول الذين يواجهون هؤلاء المرضى سواء في عيادات الرعاية الأولية أو منازلهم.
وكان الهدف من هذه الدراسة استكشاف وجهات نظر مقدمي الرعاية الأولية في كيفية نظرتهم للرعاية التلطيفية ككل وكيف ينظرون إلى دورهم وصعوباته في علاج مرضى الرعاية التلطيفية وكذلك لتقييم معرفتهم في علاج الأعراض. لهذا تم إنشاء استبان مكون من 28 عنصرا لاستكشاف المعرفة الذاتية، والاتجاهات ناحية مرضى الرعاية التلطيفية المصابين بسرطان متقدم.
في الكويت، هناك أكثر من 100 مركز للرعاية الصحية الأولية موزعة على أكثر من 1000 طبيب رعاية أولية يعملون فيها. وأجريت هذه الدراسة الاستقصائية القطاعية على هذه المراكز التابعة لوزارة الصحة. وهي مقسمة إلى خمسة مناطق رئيسية (العاصمة، حولي، الفروانية، الجهرة، والأحمدي). كل منطقة لديها 10-25 مركز للرعاية الأولية. وقد تم اختيار عشوائي بسيط لا يقل عن أربعة مراكز للرعاية الأولية من كل منطقة (~ 25٪). وأدرج جميع الأطباء من كل مركز. وشملت الدراسة حوالي 25 مركز للرعاية الأولية، ووزعت حوالي 300 استمارة، وبلغ معدل الاستجابة %79.2. انتهينا من جمع البيانات،و إدخالها والمراجعة ونحن الآن في مرحلة التحليل الإحصائي وإعداد المخطوطات. وتقوم مؤسسة الكويت للتقدم العلمي بدعمنا ماليا وتقنيا وعلميا في كل مرحلة من مراحل دراستنا."

"KFAS plays a great role in increased awareness about the important of Palliative Care (PC) in Kuwait. We performed
the current research aiming to increase awareness and knowledge of primary care physicians (PCPs) in Kuwait about
PC. In Gulf region, until recently, PC was represented by relatively few specialized hospital-based PC units. In Kuwait,
PC services are still in an early stage of development. Many of PCPs try to avoid dealing with those patients under
palliative care although they used to be the first line physicians that face those patients either in in the primary care
clinics or their homes.
The aim of this study was to explore PCPs’ towards palliative care as a whole and how they perceive their own role
and its difficulties in managing patients’ assigned for PC as well as to assess their knowledge in managing common
palliative symptoms. A novel semi-structured 28-items questionnaire was constructed to explore the self-reported
knowledge, attitude, and practice towards PC in patients with advanced cancer.
In Kuwait, there are more than 100 primary care clinics distributed all over and more than 1000 primary care physicians working in them. Cross sectional survey was performed in Primary care clinics of Ministry of Health. They are
divided into five major areas (Al-Aassema, Hawally, Al-Farawanyia, Al-Jahrah, and Al-Ahmadi). Each area has 10-25
primary care clinics. Simple random selection of at least four primary care clinics from each area (~ 25%) was done.
All physicians from each clinic were included. Around 25 primary care clinics were included in the study and ~ 300
questionnaires were distributed and the response rate was 79.2%. We finished data collection, data entry and revision.
We are now in statistical analysis phase and manuscript preparation. KFAS is continuously supporting us financially,
technically and scientifically in every phase of our study"
