Implementation of Distress Assessment and Response Tool (DART) Screening Tool in Kuwait Cancer Control Center (KCCC)

"يعد استبيان تقييم الحالة النفسية والجسدية لمركز الكويت لمكافحة السرطان برنامج مهني جديد يهدف لتعزيز تقديم الرعاية الصحية النفسية والاجتماعية لمرضى السرطان. يتكون هذا البرنامج من استبيان مدرج يشتمل على استراتيجية ذاتية للمرضى. ومن المتوقع أن يحسن هذا الاستبيان من التحويل إلى الخدمات النفسية والاجتماعية المتخصصة، زيادة رضا المرضى عن مستوى الرعاية الصحية وجودة الحياة الصحية، وأن يؤدي كذلك إلى تعزيز الطرق المثلى لبرامج فحص الضيق.
مع تنفيذ هذا الاستبيان في قسم العيادات الخارجية في مركز الكويت لمكافحة السرطان للكشف عن مستويات الضيق عند مرضى السرطان في العيادات الخارجية. وقياس حالات الضيق والاكتئاب والقلق لدى مرضى السرطان، وتطوير نظام التحويل لهؤلاء المرضى إلى عيادة علم نفس الأورام ومواصلة مراقبتهم في العيادات الخارجية في مركز الكويت لمكافحة السرطان مرة واحدة كل شهرين.
تتركز الرعاية بمرضى السرطان حول المريض نفسه بدلا من التركيز حول المرض، وهو مبدأ يدعو له الناجون من المرض وكذلك المصابون به حول العالم. إن مبدأ توفير نوعية حياة أفضل ونظام رعاية متواصل ومنظم للمرضى في تطور على مدى السنوات الماضية. سيعمل الفريق بالتعاون مع المريض وعائلته لضمان أن تكون الرعاية ملائمة ومتجاوبة مع احتياجاتهم الخاصة.
في السنوات الأخيرة أخذت الرعاية المتركزة حول المريض أوجها متعددة، منها فحص مستوى الضيق وتعد هذه الخدمة هي الأولى من نوعها في منطقة الخليج العربي وعلى الأرجح على مستوى الشرق الأوسط أيضاً."

"Emotional distress is common in cancer patients and is associated with reduced quality of life, increased symptom burden, poor compliance with care, increased health care utilization and lower life expectancy. Distress screening is recommended as a standard of practice in cancer care there have been no published reports about the prevalence of psychological distress in cancer patients in Kuwait.

The Distress Assessment and Response Tool (DART) is a comprehensive emotional, physical and practical distress screening program developed at the Princess Margaret Cancer Centre that contains a survey embedded in a graded distress response program. With the collaboration between KCCC and Princess Margaret Hospital- Canada, DART screening tool will be implemented at the KCCC outpatient department to screen for distress levels in cancer patients, measure prevalence of depression and anxiety, and promote a better communication system between patients and their oncologist with greater clinical attention to their psychosocial needs.

A pilot study was done on 416 patients attending the Lymphoma outpatient clinics between July 2013 and December 2013.0ur results showed significant Psychological distress levels in this population which were identified by DART to the primary oncologist. DART was associated with more frequent clinical assessment and greater patient and staff satisfaction. It also enhanced a better patient-nurse-physician communication. Our future steps is to introduce DART as a standard clinical practice step in all the outpatient units at the Kuwait Cancer Centre."
