Knowledge and Attitude of Emergency Physicians towards Palliative Care in Kuwait

"تلعب مؤسسة الكويت للتقدم العلمي دورا كبيرا في زيادة الوعي بأهمية الرعاية التلطيفية في الكويت. لذا أجرينا البحث الحالي الذي يهدف إلى زيادة الوعي والمعرفة لدى أطباء الطوارئ في الكويت عن مرضى الرعاية التلطيفية. في منطقة الخليج، حتى وقت قريب، كانت الرعاية التلطيفية يمثلها عدد قليل نسبيا من الوحدات المتخصصة المستندة إلى المستشفيات. وفي الكويت، لا تزال خدمات الرعاية التلطيفية في مرحلة مبكرة من التطور.
قد يحاول العديد من أطباء الطوارئ تجنب التعامل مع هؤلاء المرضى على الرغم من أنهم يعدون الخط الأول الذين يواجهون هؤلاء المرضى في غرف الطوارئ وعلى الرغم من أنهم يتعاملون مع جميع مرضى الطوارئ بطريقة احترافية تتضمن في معظم الوقت الإجراءات والفحوصات الأجتياحية أو حتى التنويم في وحدات العناية المركزة لكن هذا يختلف تمامًا عن فلسفة الرعاية التلطيفية والتي تركز في هذه المرحلة على راحة المرضى وذويهم. وذلك لأن هؤلاء المرضى لديهم أعراض معقدة في معظم الحالات ويحتاجون إلى نهج مختلف في التقييم والعلاج.
لذلك كان الهدف من هذه الدراسة استكشاف وتقييم وجهات نظر مقدمي الطوارئ تجاه للرعاية التلطيفية ككل وكيف ينظرون إلى دورهم وصعوباته في علاج مرضى الرعاية التلطيفية وكذلك لتقييم معرفتهم و درجة تمكنهم من علاج أعراض هؤلاء المرضى.
لهذا تم استخدام استبان موقف ومعرفة الأطباء تجاه الرعاية التلطيفية هو استبيان تم تطويره حديثًا لتقييم موقف ومعرفة الأطباء تجاه مرضى السرطان المتقدم. سوف يتم إجراء جميع عمليات معالجة البيانات وتحليلها باستخدام البرنامج الإحصائي SPSS. تم توزيع 154 استبيان وكان معدل الاستجابة 67.53 ٪ (ن = 104). ولقد انتهينا من جمع وإدخال البيانات ومراجعتها. ونحن الآن في مرحلة التحليل الإحصائي وإعداد المخطوطات. تقوم مؤسسة الكويت للتقدم العلمي بدعمنا ماليا وتقنيا وعلميا بشكل مستمر في كل مرحلة من مراحل دراستنا."

"Palliative care (PC) services are still lacking in many countries. In Gulf region, until recently, PC was represented by relatively few specialized hospital-based PC units. In Kuwait, PC services are still in an early stage of development. Despite in most countries, end-of-life care policies aiming at home death, the acute hospital setting remains a common place of final care and death in most countries.

Emergency physicians (EPs) as part from health care professionals face those patients who have complex symptoms in most situations and need different approach in assessment and management. EPs have firsthand experience during these moments with patients and play an integral role in helping to meet patients' and families' wishes.

Due to complexity of those patients, many of ERs try to avoid dealing with them although they are used to manage with all emergencies in a professional way that includes in most of the time invasive procedures and investigations, ICU admissions which is totally differ from palliative care view that focus in this stage on patients' comfort and relief.

So, the aim of the study is to assess the knowledge and attitude of EPs toward palliative care as a whole and toward the patients assigned for PC. This could help in the future planning of EPs needs in management of patients' assigned for PC.

Patients & Methods
Cross sectional survey will be performed in all general hospitals of Ministry of Health general hospitals across Kuwait. The approval of the ethical committee of the Ministry of Health will be taken prior to the study.

A novel semi-structured 26-items questionnaire was constructed to explore the self-reported knowledge attitudes, and practices towards PC in patients with advanced cancer. All data manipulation and analysis were performed using the SPSS."
